Monday-Thursday 8:30am-3:30pm

Intake Process:

Please bring picture ID, medical records and insurance information.

Program Fees:

Accepts Medicaid

Eligibility:

Adult 18 and over - with severe mental illness

*No violent or aggressive behavior
*Client must be able to state diagnosis and goals
*Compliant with medication
*Drug free for at least 2 months (consideration for clients in treatment)
*Clients should want to attend program and work towards goals


*To be referred to Our Place, Case Managers can send the following information:
-MHASC (Our Place) Referral form, Face Sheet, Plan of Care with RPS service, RPS goals, and Dr. signature (All info needs to be updated with in the past year).
-Latest 90 day progress summary or clinical equivalent.
