Intake Process:

Call, walk-in, or schedule an appointment online.

Applications for sliding scale payments can be found on website or in person.
Documents needed:
*utility bill for proof of address
*pay stubs from last 4 weeks for all household members
*statement from employer showing last month full income for all household members
*copy of latest tax return
*copy of current year Social Security Award Letter (if applicable)
*copy of benefits eligibility statement (if applicable)
*copy of financial aid statement (if applicable)
*copy of child support court order or child support check (if applicable)

Program Fees:

Accepts Medicare, Medicaid, and most private insurance.
Sliding scale payments are available to uninsured patients (or patient with Medicare or Medicaid for certain services) with a total household income at or below 200% of federal poverty guidelines.

Languages:

English and Spanish

Eligibility:

Residents of Orangeburg, Bamberg, Calhoun, and Upper Dorchester counties.

Patients with no insurance with a total household income at or below 200% of federal poverty guidelines qualify for sliding scale payments.
Additionally, patients with Medicare or Medicaid may qualify for sliding scale payments for services not covered by Medicare or Medicaid.
