A regional office of Community Long Term Care (CLTC) of the SC Department of Health and Human Services (DHHS).
This statewide program provides:
*mandated federal/state pre-admission screening for mental illness and the mentally handicapped
*those seeking nursing home placement
*helps individuals who want to live at home
*individuals who need assistance with their care and are financially eligible for Medicaid
Hours:
Administrative Hours: Monday - Friday 8:30am-5:00pm
Intake Process:
Call to complete referral.
Documents needed: photo ID, and Medicaid information.
*** All referrals must go through Centralized Intake for processing. Call (855)278-1637 or Fax (803)255-8340. Electronic Referral may be accessed via website***
