Monday, Tuesday, Thursday 8:00am-5:00pm
Wednesday 9:30am-6:30pm

Intake Process:

Call for appointment or Walk-in appointments are available as the schedule allows.
Please plan to arrive 30 minutes before your appointment time to complete necessary forms.
Bring all current medications.

Registration is required on or before your first visit.
*State license, ID, and Social Security Card
*Private Insurance, Medicare, Medicaid, Other
*Proof of Address (Phone bill, Utility bill, etc.)
*Proof of Income (If requesting discount)

Bring the following items to your first appointment:
*Proof of unemployment; food stamps; housing; and/or child support
*Social Security Income Statement
