Services are only for Individuals with Sickle cell or family member of someone with sickle cell.
Agency provides:
*genetic screening and counseling
*case management
*client education
*adult and parent support group
*children support group
*electric bill assistance (for sickle cell patients, as funds are available)
*medical transportation (for sickle cell patients, as funds are available)
*water bill assistance (for sickle cell patients, as funds are available)
*hospital visitation
*referral

Monday-Friday 9:00am-5:00pm

Intake Process:
Call or walk in.
Call Yvonne Donald (800-506-1273) for assistance establishing Nurse Case Management.
Nurse Case Management is required to qualify for emergency assistance with electric bills, water bills, and for medical transportation.
Documents needed:
Medical record and laboratory proof of sickle cell disease (Nurse Case Management only)
Adult support groups are for those with sickle cell disease and the adult persons in their lives.
Youth support groups are for children ages 5-12 with sickle cell disease.

Individuals with Sickle cell or family member of someone with sickle cell residing in the Midlands or Upstate.
