4. Unfunded patients discharged from local hospitals without financial means for medical follow-up.
5. Hypertension
6. Pharmacy/ Lab Services. Patients who are eligible for care are also eligible to receive medications from their pharmacy as well as lab testing and supplies.

Hours:

Monday, Tuesday, Thursday 8:00am - 5:00pm

Intake Process:

Call for appointment and to fill out application including income statement.
When you call you will hear a recording that prompts to leave a message on the "new patient appointment line" explaining your needs. If you do not receive a return call within one week to schedule an appointment, that indicates they are full and you will need to seek healthcare services elsewhere.

Spanish on Tu 8:30am - 12:00pm

Eligibility:

Adults and youth between the age of 18 to 65.
Harvest Free Medical Clinic patients must meet the following eligibility criteria:

No insurance, including Medicare, Medicaid, or veterans benefits
Household income less than 200% of the federal poverty level
Place of residence does not affect eligibility
Patients who are eligible for care are also eligible to receive medications from our pharmacy as well as lab testing and supplies.

Accepting new patients based on eligibility with the following prioritization of services:

Complex medical problems such as heart failure, lupus, asthma, etc.
Mental health disorders
Type 1 Diabetes
Unfunded patients who are discharged from local hospitals without financial means for medical follow-up
Hypertension
